To register for the 2011-2012 ice hockey season, please complete ALL of the following steps and bring all paperwork to the first practice:

STEP 1  Bring copy of player's Birth Certificate

STEP 2  Register with USA Hockey (players and coaches).  Print Confirmation page.

STEP 3  Print and Complete ALL Forms below:

STEP 4  VYHA Registration
Complete the form at the bottom of the page.
Pay league dues.  [You may pay dues online or bring a check made payable to VYHA to the first practice.  Register for the House Team before September 15, 2011 and your cost will be $500 per player (a $50 savings).  Register for the Travel Team before December 1, 2011 and your cost will be $450 (a $50 savings).]
If you do not wish to pay the Registration fee in full, we offer the ability to make partial payments.

*It is important to note that players will not be allowed on the ice without ALL 4 steps completed.  Forms must be received by the team manager or league registrar prior to the player stepping onto the ice.
